consequences that do not work experience opportunity cost

I have kept an appointment on personal experience, which reads:

"The experience: You do not learn from it. Because the most important consequences of the acts occur in another system and eventually return to create the same problems that are attributed to the actions of others. " (senge. 69)

is in a language too technical perhaps, but the meaning is not well understood, we take the work of translating it (in my words)

"Men in certain circumstances do not learn experience because experiencing consequences (of their actions) occur in other areas away from the initial facts. And therefore the cause and effect have no clear link.
